Subject: Complaint Against ICICI Bank for Unauthorized Debit Card Issuance and Wrongful Deduction of Charges


I am writing to formally lodge a complaint against ICICI Bank regarding deficiency in service and unauthorized charges levied on my bank account (Account Number: ******** 6350).

I wish to bring to your kind notice that the bank has issued not one but two debit cards on my account without my knowledge, request, or consent. Subsequently, annual charges have been levied on both cards without my authorization.

The details of the unauthorized debit cards and charges are as follows:

  • Picture Debit Card: **********20 — Annual Fee: ₹599 + 18% GST
  • Debit Card: **********96 — Annual Fee: ₹300 + applicable GST
I would like to clearly state that I have never applied for or consented to the issuance of these debit cards. I already possess a valid and active debit card linked to my account. Therefore, issuing additional debit cards and levying charges without my explicit approval is completely unjustified and amounts to an unfair trade practice.

Furthermore, this account was originally opened in connection with a loan, and the bank’s act of adding chargeable services without consent demonstrates a clear deficiency in service, lack of transparency, and violation of customer rights.

Despite being a responsible customer, I have been subjected to unauthorized financial deductions and unnecessary mental harassment due to the negligent and arbitrary actions of the bank.

I respectfully seek the following reliefs from this Honorable Commission:

  1. Direction to ICICI Bank to immediately cancel both unauthorized debit cards issued on my account.
  2. Full refund of all charges levied (₹599 + +18%^ GST and ₹300 + +18%^ GST) till todays date.
  3. Direction to the bank to ensure that no chargeable products or services are issued on my account without my explicit consent in the future.
I request this Commission to take appropriate action against the bank and provide necessary relief at the earliest.
