**Respected Sir/Madam,**
I am Amit Daljeet Yadav, resident of Indira nagar Sanjay Gandhi maidan shree krishna rahiwashi chawl wagle estate thane west 400604 Maharashtra , would like to file a complaint against Care Health Insurance Ltd. for deficiency in service and unfair trade practices.
I had purchased a health insurance policy from Care Health Insurance with the following details:
* Policy Number: 85497617
* Policy Holder Name: Daljeet Yadav
* Date of Policy: 20 jun 2024
On 05 March 2026, I was hospitalized at JNANESHWARI BRAIN AND SPINE CENTRE AND ACCIDENT HOSPITAL due to Spine problem back pain. I incurred medical expenses amounting to ₹44,486. After discharge, I submitted all required documents for reimbursement claim on 11 March 2026.
Despite submitting all necessary documents, the company has:
* Wrongfully rejected my claim /
* Delayed the claim without valid reason /
* Not responded properly

I have contacted their customer support multiple times, but I have not received any satisfactory response. This has caused me financial loss and mental harassment.
This act clearly amounts to **deficiency in service** and **unfair practice** under the Consumer Protection Act.
**I therefore request the Honorable Court to:**
1. Direct the company to settle my claim amount of ₹ 44.486 immediately
2. Provide compensation for mental harassment and inconvenience
3. Reimburse any additional costs incurred due to delay
4. Take appropriate action against the company

* Claim submission proof
* Email/communication records
