Sir/Madam,

Myself Dr. Syed Vasiullah from Chennai writing this to complain regarding an erroneous reports made by Aarthi Scans and labs Chennai.

My wife, Mrs. Naziya Nousheen, is a 35 year old pregnant woman and a known case of distal renal tubular acidosis (distal RTA) on regular follow‑up with a nephrologist. In distal RTA, patients typically have metabolic acidosis with low bicarbonate and a tendency to hypokalemia, often requiring potassium supplements.

In November, during a routine check her serum electrolytes showed very low bicarbonate, and she was started/continued on bicarbonate supplementation with advice to repeat electrolytes after one month.

On 14-12-2025, we visited the Kilpauk branch of Aarthi scans and labs for serum electrolytes (Na, K, HCO₃⁻, etc.) as the follow‑up test. The first report from the lab showed potassium 6.2 mEq/L (told to me orally over phone) and bicarbonate 22 mEq/L, i.e., apparently normal bicarbonate but dangerously high potassium.The staff, acknowledging the seriousness, asked for a repeat sample. We provided a second sample on 22-12-2025 as home collection.

The second report on 23-12-2025 showed potassium 7.3 mEq/L, again grossly elevated. These values are completely inconsistent with her known distal RTA status (where hypokalemia is expected) and with her clinical condition; however, given that such high potassium levels even after 2 samples, can be life‑threatening, we panicked and rushed to our treating nephrologist at MMM Hospital. The nephrologist urgently repeated serum electrolytes in the hospital laboratory. Approximately 2 hours later the hospital report showed potassium 3.6 mEq/L (normal) and bicarbonate 11 mEq/L (still low), indicating that your reports had falsely elevated potassium and an inappropriately normal bicarbonate, thereby masking the ongoing metabolic acidosis.

Because of the erroneous reports from Aarthi scans and labs, we underwent:
Severe mental agony and fear for the life of a pregnant patient. Unnecessary emergency hospital visit and consultation. Additional laboratory charges at the hospital and travel expenses and physical stress for a pregnant woman with distal RTA.

Our nephrologist questioned us what if we had visited some other nearby hospital and treated for hyperkalemia based on this report? It would have been fatal, which further shook our confidence.

I immediately called up their Operations manager and the AGM over the phone and raised my concern/complaint on 24-12-2025 and a written complaint was sent on 25-12-2025, but still there is no reply or resolution on this.

In view of the above, I requested the following:
a. A written explanation of how such gross errors in potassium and bicarbonate values occurred in two consecutive samples on the same day.
b. A copy of their internal quality control (QC) records for electrolytes (particularly potassium and bicarbonate) for the dates surrounding [15-12-2025 & 22-12-2025].
c. Full refund of all test charges for the erroneous reports.
d. Compensation of Rs 500000 towards: Emergency consultation and test expenses at [Madras Medical Mission Hospital], travel expenses, mental agony and physical stress caused to a pregnant woman and her family.
e. Written corrective assurance regarding your QC processes to prevent recurrence with other patients.

Kindly request you to take necessary action on this.
