We have already submitted multiple grievances: two for the first claim (on 19 May 2025 and 27 May 2025) and three for the second claim (on 05 Nov 2025, 21 Nov 2025, and 03 Dec 2025).
An affidavit addressing an ID mismatch was submitted previously for the first claim and subsequently used for the third claim. However, the second claim was rejected and closed due to this same ID mismatch. We requested that the file be opened to allow the submission of the affidavit, but we have received no reply, nor is there an option to submit this document online through the portal.
Hence, I humbly request that you review the details provided and approve the outstanding claim amounts favorably.
